Upcoming Appointments
If you have an upcoming appointment please make sure your patient portal information is complete prior to your appointment.
If you have not received an email regarding your patient portal, please call our office so that it can be activated.
No-show Policy: Patients who do not show up for their scheduled appointments or who do not cancel or reschedule at least 24 hours prior to their appointment will be billed $50 for regular visits and a $150 no show fee for missed Cosmetic and Laser appointments. All surgery appointments will be assessed a $250 no show fee. Exceptions can be made in certain circumstances. Please notify our office as soon as possible if you cannot keep your appointment.

Fees charged in this office are comparable to those charged by other specialists who are competent in dermatology and the services we provide. We have contracts with many providers. However, contractual agreements are subject to change.
We recommend that you contact your carrier before your appointment to verify that services provided by your physician here are covered by your insurance. We are contracted with the following:

We bill any non-contract PPO insurance. However, any difference between the amount paid by the carrier and the actual fees for providing the service are the patient’s responsibility.
Statements are generally sent seven (7) to 30 days after your visit, depending on your insurance coverage. This varies significantly for certain types of accounts, so please do not be concerned if it takes longer to receive our statement. Payment is required in full upon receipt of your statement.
Uninsured patients are required to pay at the time of service. If a procedure is done which requires a delay in full charges, partial payment is required.
Payment for Cosmetic Services
In this office, some procedures are classified as cosmetic corrections. Fees for these procedures are ordinarily paid for in advance.

What insurances do you take?
We take most commercial insurance Please see our insurance list. Unfortunately, we do not participate with any AHCCCS plans.
Do you give any discount for self-pay patients?
We do not reduce charges for procedures, services or products that are paid for out-of-pocket.
How much is an office visit?
Office visit charges vary depending on the time spent with the provider and whether any procedures are done. You will be quoted a price prior to any procedure being done.
Do you take walk-ins?
We do not formally take walk-ins, but same-day appointments are often available. Call during office hours to check for appointment availability or click here.
Do you perform the Mohs procedure?
Yes, Dr. Juracek is our Mohs surgeon. He performs Mohs surgery at our Arrowhead and Surprise office locations.
Will Medicare cover more than one full body skin check a year?
Yes. If you have a history of skin cancer, sun damage or other medical conditions, Medicare will cover additional exams in a year if you or your doctor feel that they are medically necessary.
What cosmetic fillers do you sell and what are the prices?
We carry several cosmetic fillers. Prices vary, Please visit our Products page for the list of all of our cosmetic fillers.
Can a Physician Assistant (P.A.) treat me the same way a doctor can?
Yes.
Can you call my primary care physician (PCP) and get a referral for me?
Unfortunately, referrals need to be initiated by the PCP. Referrals can be faxed to any of our three locations.
